В статье обсуждаются трудности диагностики и терапии пустулезного псориаза у ребенка на фоне рецидивирующего дерматомикоза, а также иммунопатогенетические механизмы пустулезного псориаза, течение и прогноз. Описание клинического случая. Описан случай заболевания инфильтративно-нагноительной формой микроспории волосистой части головы у ребенка 11 лет, длительно страдающего вульгарным псориазом с его трансформацией в генерализованный пустулезный псориаз (тип Цумбуша). Необходимость назначения иммуносупрессивной терапии по поводу тяжелой формы псориаза диссонировала с грибковой инфекцией кожи, при которой иммунная супрессия приводила к рецидивирующему течению последней. Заключение. Комбинированное лечение (системные глюкокортикостероиды, метотрексат, антибиотики и гризеофульвин) позволило добиться клинико-лабораторного выздоровления в отношении микроспории у ребенка с генерализованным пустулезным псориазом.</p></abstract><trans-abstract xml:lang="en"><p>Background. This article discusses issues of diagnosis and management of pustular psoriasis in a child associated with recurrent dermatomycosis, immunopathogenetic mechanisms of pustular psoriasis, its course and prognosis. Clinical Case Description. The clinical case of infiltrative-suppurative form of scalp microsporia in 11 years old child with long-term psoriasis vulgaris and its transformation to generalized pustular psoriasis (von Zumbusch) is described. The need of immunosuppressive therapy prescription for severe psoriasis was challenged with fungal skin infection as immune suppression would lead to its recurrent course. Conclusion.
