Основным методом хирургического лечения ходьбы на носках принято считать удлинение ахиллова сухожилия, которое ассоциировано с риском чрезмерной коррекции, разрывом сухожилия или его плохим кровоснабжением, нарушением подошвенного сгибания. Альтернативой этому методу может быть малоинвазивная методика — миофасциотомия.</p><p>Цель исследования — изучить результаты перкутанной миофасциотомии у пациентов с идиопатической ходьбой на носках с выраженной деформацией стоп и отсутствием эффекта от консервативной терапии.</p></sec><sec><title>Методы</title><p>Методы. В исследование включали данные (сведения из медицинской документации) пациентов с идиопатической ходьбой на носках, которым была проведена перкутанная миофасциотомия по методу Ульзибата. В числе исходов лечения оценивали частоту сохранения (рецидива) ходьбы на носках на следующий день и через 12 мес после операции. Дополнительно анализировали объем движений в голеностопном суставе, наличие деформаций стопы, удовлетворенность родителей результатами операции, а также количество послеоперационных осложнений.</p></sec><sec><title>Результаты</title><p>Результаты. В период исследования перкутанная миофасциотомия была выполнена у 50 детей (11 женского пола), медиана возраста 13 (10; 16) лет. На следующий день после оперативного вмешательства ходьба на носках отсутствовала у всех пациентов. В течение 12 мес после операции рецидив ходьбы на носках отмечен у 3 (6%) пациентов. При оценке подвижности стопы на следующий день после операции и через 12 мес у всех пациентов показатели дорсифлексии достигли 10 градусов. Среди осложнений оперативного вмешательства отмечено только формирование послеоперационной гематомы — у 10 (20%) пациентов. Родители 3 (6%) детей (все с рецидивом ходьбы на носках) остались недовольны результатами операции.</p></sec><sec><title>Заключение</title><p>Заключение. Перкутанная миофасциотомия по Ульзибату показала высокую эффективность и относительную безопасность в лечении пациентов с идиопатической ходьбой на носках.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. The main method of surgical treatment of toe walking is Achilles tendon extension, which is associated with the risk of excessive correction, tendon rupture, its poor blood supply, plantar flexion deformity. Another alternative is minimally invasive technique, myofasciotomy.</p></sec><sec><title>Objective</title><p>Objective. The aim of the study is to study the results of percutaneous myofasciotomy in patients with idiopathic toe walking with severe foot deformity and with no effect of conservative therapy.</p></sec><sec><title>Methods</title><p>Methods. The study included data (medical records) from patients with idiopathic toe walking who underwent Ulzibat percutaneous myofasciotomy. We have evaluated the frequency of persistence (recurrence) of toe walking on the next day and 12 months after surgery among all the treatment outcomes. Additionally, the range of motions in ankle joint, the presence of foot deformities, parental satisfaction with surgery results, as well as the number of postoperative complications were analyzed.</p></sec><sec><title>Results</title><p>Results. Percutaneous myofasciotomy was performed in 50 children (11 females) with median age of 13 (10; 16) years during the study period. There were no cases of toe walking on the next day after surgery. Recurrence of toe walking was revealed in 3 (6%) patients within 12 months after surgery. Dorsiflexion rates reached ≥ 10 degrees in all patients at evaluating foot mobility on the next day after surgery and 12 months later. The only revealed surgical treatment complication was postoperative hematoma in 10 (20%) patients. Parents of 3 (6%) children (all with recurrent toe walking) were dissatisfied with surgery results.</p></sec><sec><title>Conclusion</title><p>Conclusion.
