Заболевание оказывает выраженное негативное влияние как на качество жизни, так и на психосоциальное благополучие, приводя к формированию ряда психических расстройств и физическому дискомфорту, включающему в себя не только типичные клинические проявления, но и симптомокомплекс постакне. В настоящее время центральное значение в патофизиологии заболевания занимает изменение микробиома кожи, происходящее в результате взаимодействия генетических и экспосомальных факторов, ведущих к повышенной выработке кожного сала себоцитами и сопутствующей дисфункции эпидермального барьера на фоне протекающего воспалительного процесса. В настоящее время варианты лечения ВА предусматривают местную и системную терапию, включающую в себя ряд клинико-фармакологических групп, однако большинство имеющихся в арсенале дерматовенерологов терапевтических опций могут спровоцировать сухость кожи, а также вызывают раздражение чувствительной кожи подростков в связи с дополнительным нарушением эпидермального барьера и видового состава микробиома, что значимо снижает приверженность пациента лечению, а значит, и уменьшает вероятность успеха терапевтической интервенции. Стоит отметить, что лечение ВА проводится в течение длительного времени с целью предотвращения рецидива заболевания, наблюдающегося в одной трети случаев, что подчеркивает важность соответствующей оптимизации схем лечения, использующихся в широкой клинической практике. В данном случае ведущее значение имеют рекомендации по уходу за кожей, который является необходимой частью лечения ВА и может не только улучшить приверженность пациента терапии, но и повысить эффективность назначенных лекарственных средств за счет формирования синергического взаимодействия. Тем не менее, для обеспечения подобного эффекта требуется провести тщательный разбор имеющихся средств для базового ухода, проанализировав их состав и определив влияние содержащихся ингредиентов на структурно-функциональную целостность кожи.</p></abstract><trans-abstract xml:lang="en"><p>Acne vulgaris (AV) is a common skin pathology in adolescents, it is characterized by chronic recurrent inflammatory process affecting the pilosebaceous units. This disease has significant negative effect on both quality of life and psychosocial well-being, resulting in development of various mental disorders and physical discomfort, including not only typical clinical signs, but also post-acne symptom complex. Nowadays, the paramount importance in disease pathophysiology are changes in the skin microbiome caused by interactions between genetic and exposomal factors leading to increased cutaneous fatty secretion by sebocytes and concomitant dysfunction of the epidermal barrier due to ongoing inflammatory process. Current AV treatment options include topical and systemic therapies with various clinical and pharmacological groups. However, most available options (for dermatovenerologists) can provoke dry skin and irritate sensitive adolescent skin due to additional disruption of epidermal barrier and microbiome composition. All together it significantly reduces patient compliance, and, therefore, reduces the treatment success rate. We have to mention that AV management should be carried out for a long time to prevent the recurrence observed in one third of all cases. It emphasizes the topicality of adequate optimization of treatment regimens used in wide clinical practice. In this context, guidelines for skin care have the leading role. It is crucial part of AV management, and it can not only improve compliance, but also increase the efficacy of prescribed drugs through synergistic interactions.
