Атопический дерматит (АтД) — хроническое воспалительное заболевание кожи, развивающееся на фоне генетически обусловленных нарушений кожного барьера и нарушений функционирования иммунной системы. В последние годы в структуре случаев АтД отмечено увеличение доли больных с аутоиммунными заболеваниями (АИЗ). Цель исследования — изучить частоту случаев АИЗ среди детей, госпитализированных с АтД. Методы. В исследование включали пациентов с АтД средней или тяжелой степени течения, госпитализированных в специализированное отделение стационара в период с ноября 2015 по апрель 2025 г. Наличие АИЗ и аллергических заболеваний определяли по данным медицинской документации. Результаты. В исследование включены 10 239 пациентов с АтД. Сопутствующее заболевание ( 1) диагностировано у 8103 (79,1%) детей, у 6475 (63,2%) пациентов — аллергические заболевания. Хотя бы одно АИЗ диагностировано у 1628 детей (15,9%; 95% ДИ 15,3–16,7%), чаще всего устанавливали наличие гнездной алопеции, целиакии и витилиго. Заключение. Обследование пациентов с АтД средней или тяжелой степени должно включать в себя оценку симптомов и иммунологических маркеров АИЗ. Общие патогенетические механизмы позволяют формировать единую стратегию раннего выявления АтД и АИЗ и наблюдения за пациентами из группы высокого риска.</p></abstract><trans-abstract xml:lang="en"><p>Background. Atopic dermatitis (AD) chronic inflammatory skin disease developing associated with genetically determined skin barrier disorders and immune system impairment. In recent years, there is an increase in the proportion of patients with autoimmune diseases (AIDs) among AD cases. Objective. The aim of the study is to examine AIDs incidence among children hospitalized with AD. Methods. The study included patients with moderate or severe AD hospitalized in specialized hospital department from November 2015 to April 2025. AIDs and allergic diseases presence was determined according to medical records. Results. The study included 10,239 patients with AD. Comorbidities (1) were diagnosed in 8103 (79.1%) children, allergic diseases — in 6475 (63.2%). At least one AID was diagnosed in 1,628 children (15.9%; 95% CI 15.3–16.7%). The most common were alopecia areata, celiac disease, and vitiligo. Conclusion. Examination of patients with moderate or severe AD should include evaluation of AIDs symptoms and immunological markers.
