Синдром Нетертона — генетическое заболевание, для которого характерно сочетание ихтиозиформной эритродермии, аномалии роста волос, а также тяжелого атопического дерматита с высокой концентрацией общего IgE в сыворотке крови. Однако помимо описанной «триады», у пациентов с синдромом Нетертона описаны отставание в росте и физическом развитии. В большинстве случаев это связано с тяжелым течением основного заболевания, но в некоторых ситуациях причиной низкорослости является гипопитуитаризм с дефицитом соматотропного гормона. Описание клинического случая. У пациента с подтвержденным диагнозом синдрома Нетертона при госпитализации отмечены отставание в физическом развитии и низкорослость, а также тяжелое течение основного заболевания с развитием ихтиозиформной эритродермии. Учитывая тяжелое течение синдрома Нетертона, пациенту по показанию off-label была инициирована терапия ингибитором IL-4/13 дупилумабом. При дополнительном обследовании выявлен сопутствующий гипопитуитаризм с изолированным дефицитом соматотропного гормона. Назначена заместительная терапия соматотропином. В результате комбинированной терапии удалось добиться значительного улучшения состояния пациента. Клинически значимые нежелательные явления на фоне терапии не зарегистрированы. Заключение. Отставание в физическом развитии пациентов с синдромом Нетертона является основанием для проведения исследований причин низкорослости. Комбинация заместительной терапии соматотропным гормоном с применением генно-инженерного биологического препарата дупилумаб является эффективной и безопасной, однако требует междисциплинарного подхода к ведению таких пациентов.</p></abstract><trans-abstract xml:lang="en"><p>Background. Netherton syndrome is a genetic disease characterized by combination of ichthyosiform erythroderma, hair growth abnormalities, and severe atopic dermatitis with a high total IgE levels in the blood serum. However, beside the described “triad", patients with Netherton syndrome can have growth and physical development delays. In most cases, it is associated with severe course of the underlying condition, but in some cases, the cause of short stature is hypopituitarism with somatotropin deficiency. Case description. The patient with confirmed Netherton syndrome was hospitalized with physical development delay and short stature, as well as a severe course of the underlying disease with ichthyosiform erythroderma development. Patient was administered with IL-4/13 inhibitor, dupilumab, according to the off-label indication due to severe Netherton’s syndrome course. Additional examination has revealed comorbid hypopituitarism with isolated somatotropin deficiency. Growth hormone therapy was prescribed. As a result of such combined treatment, we have achieved significant improvement in the patient’s condition. No clinically significant adverse events have been reported during therapy. Conclusion. Physical development delay in patients with Netherton syndrome is the reason for conducting research on the short stature causes.
