Инфантильная полушарная глиома (ИПГ) — глиальная опухоль, развивающаяся внутриутробно или в младенчестве, часто больших размеров. Своевременная диагностика опухоли затруднена по причине ее редкости у детей раннего возраста, а также вследствие вариативности молекулярно-генетических характеристик опухоли. Применение предоперационной и интраоперационной нейровизуализации при невозможности тотальной резекции необходимо для удаления морфологически значимых участков опухолевой ткани, а цитогенетическое исследование — для постановки окончательного диагноза.</p><p>Описание клинического случая. Ребенок с задержкой внутриутробного развития и церебральной ишемией 2-й степени, синдромом двигательных нарушений. При нейросонографии в возрасте 13 сут выявлено объемное кистозное образование, которое не было определено как опухоль. В возрасте 2,5 мес отмечено увеличение окружности головы на 7 см, по данным нейросонографии определено увеличение кистозной полости в левой гемисфере, в связи с чем выполнено вентрикулоперитональное шунтирование. В возрасте 8 мес в результате магнитно-резонансной томографии головного мозга выявлено объемное кистозно-солидное новообразование в левой гемисфере. С целью постановки диагноза и определения тактики лечения была применена комбинированная нейронавигация: на предоперационном этапе — позитронно-эмиссионная томография с [18F]-фторэтилтирозином, совмещенная с компьютерной томографией, во время операции — навигация под ультразвуковым и флуоресцентным контролем. Выполнено цитогенетическое исследование опухоли, подтвердившее глиальный характер новообразования.</p></sec><sec><title>Заключение</title><p>Заключение. Показана сложность диагностики ИПГ, а также польза предоперационной и интраоперационной нейронавигации с последующим цитогенетическим исследованием для установления окончательного диагноза.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Infantile hemispheric glioma (IHG) is a glial tumor (often large sized) that often develops in utero or during infancy. Timely diagnosis is challenging due to its rarity in young children and the variability of its molecular genetic characteristics. Implementation of preoperative and intraoperative neuroimaging allows to remove the most morphologically significant areas of tumor tissue when total resection is not possible, while cytogenetic analysis allows for a definitive diagnosis.</p></sec><sec><title>Case description</title><p>Case description. The child with intrauterine growth retardation has grade 2 cerebral ischemia and movement disorders syndrome. Neurosonography at 13 days of age has revealed large cystic lesion that was not identified as a tumor. An increase in head circumference up to 7 cm was noted at the age of 2.5 months, while neurosonography has shown enlargement of the cystic cavity in the left hemisphere, thus ventriculoperitoneal shunting was performed. Magnetic resonance imaging of the brain has revealed a large cystic and solid lesion in the left hemisphere at the age of 8 months. We used combined neuronavigation to establish the diagnosis and determine treatment tactics: preoperative positron emission tomography with [18F]fluoroethyl-l-tyrosine combined with computed tomography and ultrasound and fluorescence guidance during the surgery. Tumor cytogenetic analysis was performed and confirmed the glial nature of the neoplasm.</p></sec><sec><title>Conclusion</title><p>Conclusion.
