Особенности поражения ЛОР-органов у детей с муковисцидозом

Аннотация

Обычный подход к ведению пациентов с муковисцидозом не предполагает оценку состояния верхних дыхательных путей, хотя патологические изменения, являющиеся последствием мутации гена муковисцидозного трансмембранного регулятора проводимости, в равной степени затрагивают слизистую оболочку полости носа и околоносовых пазух. Примерно у половины пациентов с муковисцидозом имеет место хронический риносинусит и/или полипы носа, что ухудшает течение и без того тяжелого заболевания. Хронические гиперпластические процессы в околоносовых пазухах обширны, нередко рецидивируют, могут приводить к деструктивным процессам в костных стенках пазух, перегородке носа. При этом увеличивается число госпитализаций и койко-дней за одну госпитализацию. Малая осведомленность оториноларингологов в поликлиниках и стационарах о патологии ЛОР-органов при муковисцидозе приводит к несвоевременной диагностике, излишним манипуляциям, неэффективным методам лечения, в том числе к оперативным вмешательствам. Все это диктует необходимость ранней скрининг-диагностики и разработки адекватных методов лечения ЛОР-осложнений муковисцидоза, как консервативных, так и хирургических, с отработкой четких показаний, а также противопоказаний к ним.
