ОПЫТ ПЕРЕКЛЮЧЕНИЯ НА ВТОРОЙ ИНГИБИТОР TNF ПАЦИЕНТКИ С ТЯЖЕЛЫМ ТЕЧЕНИЕМ ЮНОШЕСКОГО ПОЛИАРТРИТА И РЕЗИСТЕНТНОСТЬЮ К ПЕРВОМУ TNF-БЛОКАТОРУ1

Аннотация

Представлен  случай тяжелого прогрессирующего течения полиартикулярного ювенильного идиопатического артрита, дебютировавшего в раннем возрасте, рефрактерного  к терапии метотрексатом, циклоспорином и глюкокортикостероидами. Клинический пример демонстрирует, что переключение на второй ингибитор фактора некроза опухоли альфа в случае развития вторичной резистентности  к первому препарату из этой группы обеспечивает восстановление терапевтического  эффекта и не сопровождается серьезными нежелательными явлениями. Через 6 мес после переключения на адалимумаб вследствие «ускользания» эффекта этанерцепта у пациентки купировались воспалительные изменения  и значительно  увеличился  объем  движений  в  суставах,  нормализовались лабораторные  показатели активности болезни. Через 1 год была констатирована ремиссия заболевания. Ребенку удалось полностью отменить глюкокортикостероиды для  перорального  приема. Длительность ремиссии суставного  синдрома составила  3 года. Нежелательные явления были несерьезными и не стали основанием для отмены препарата.
