Филаггрин и атопический дерматит: клинико-патогенетические параллели и возможности терапевтической коррекции

Аннотация
Атопический дерматит (АтД) представляет собой многофакторное воспалительное заболевание кожи, одним из элементов патогенеза которого является дисфункция эпидермального барьера. Раннее развитие АтД ассоциировано с нарушением функции филаггрина — белка, участвующего в агрегации кератиновых филаментов в верхних слоях эпидермиса и удержании липидов и белков между корнеоцитами. Зачастую нарушение функции филаггрина сопровождается присоединением вторичной инфекции и высоким риском развития других аллергических заболеваний. Происходит это по причине нарушения процесса терминальной дифференцировки клеток эпидермиса и, как следствие, целостности кожного барьера. В связи с этим основой терапии АтД является длительное регулярное использование эмолентов. Более эффективного лечения пациентов с АтД удалось добиться с внедрением в практику нового класса эмолентов («эмоленты плюс»), снижающих активность воспалительных процессов в коже за счет замещения структурных компонентов нарушенного эпидермального барьера.
