АНТИ-CD20 МОНОКЛОНАЛЬНЫЕ АНТИТЕЛА В ТЕРАПИИ ГЕМОЛИТИЧЕСКОЙ АНЕМИИ У ДЕВОЧКИ-ПОДРОСТКА С СИСТЕМНОЙ КРАСНОЙ ВОЛЧАНКОЙ

Аннотация
Гемолитическая анемия встречается в 10% случаев системной красной волчанки у детей с гематологическими изменениями. Для лечения данного состояния применяют высокие дозы глюкокортикоидов. Препаратами второй линии при рефрактерном течении заболевания являются азатиоприн, микофенолата мофетил, циклоспорин и циклофосфамид. Ритуксимаб — анти-CD20 моноклональное антитело — используют для лечения системной красной волчанки, рефрактерной к стандартной иммуносупрессивной терапии. В данной статье представлен случай успешной терапии гемолитической анемии у девочки-подростка с системной красной волчанкой. Назначение ритуксимаба позволило избежать повышения дозы глюкокортикоидов и назначения циклофосфамида, а также предотвратить развитие тяжелых нежелательных явлений от терапии высокими дозами этих препаратов. Однако для изучения продолжительности эффекта, безопасности терапии, подбора его оптимальной дозы необходимы данные многоцентровых рандомизированных контролируемых исследований.
