The article presents the study of immunization with DTP vaccine (diphtheria, pertussis (whooping cough) and tetanus) in children from mothers with HIV and immunization against measles in 31 of them. The postvaccinal period was uneventful in most cases. The frequency of inter current illnesses (immunization complications) has been comparable with that in sickness prone children. The differences in the mean geometric values of antibody titresto diphtheria and measles between the HIV apositive children (group B23) and the hivanegative children from the mothers with HIV (group R75) have not reached the statistically significant levels. Meanwhile, the structure of measles antibody titre has shown that in HIV apositive children the protective antibodies were absent or reached the minimal level (1:5).

Strong correlation between characteristic features of norms ECG intervals and age and gender of children has been established due to the process of growth and development. These findings should be regarded in functional identification of heart conduction disorders. The ECG waves and intervals become stable at an age of 13–14 years due to the maturity of heart conduction system.

The purpose of this open study was to evaluate efficacy and safety of infliximab, an antiatnfa ?monoclonal antibody, in 37 children with juvenile idiopathic arthritis (JIA). Group 1 comprised 16 patients with severe longaterm jia with systemic onset. Group 2 comprised 21 patients with oligoa and polyarticular JIA. Groups were matched in demographic features. The follow up period has ranged between 6 weeks and 2 years. All children received infliximab and concomitant D mard therapy, 3 of them received oral glucocorticoids. Inflixa imab with a mean dose of 6,5 ± 0,3 mg/kg was administered intravenously at baseline, 2, 6 and every 8 week. Infliximab has shown statistically significant improvement (p < 0,01) in all patients with early oligoa and polyarticular JIA (group 2). Remission following the second infusion (at the 3 week of observation) has been registered in all patients with oligoa and polyarticular jia, meanwhile in patients with systemic JIA remission has been registered only in 2 cases at a time of 7 months of treatment with infliximab. Eight (50%) patients from group 1 and 2 children (9,5%) from group 2 discontinued infliximab due to inefficacy. 5 (24%) of patients from group 2 developed side effects that led to infliximab discontinuation. Therefore, the therapy with infliximab has proved to be sound, safe and effective with best results in patients with oligoa and polyarticular jia.

LITERATURE REVIEW
The article reflects current viewpoints of Russian and foreign scientists on a problem of pathology of primary headaches (migraine and tension headache) in children, it's frequency in adults and children, diagnostic aspects and characteristic features of clinical course in children. Much attention is paid to the role of nitric oxide in pathogenesis of primary headaches.

The article provides literature survey of health related quality of life (HRQL) in patients with gastrointestinal tract (GI tract) disorders focusing on it's application in pediatrics. The review defines the term of healtharelated quality of life and presents the HRQL questionnaires specially developed for patients with different GI tract disorders indicating possible areas of their use. The article summarizes the reports assessing the healtha related quality of life in adults with GI disorders and disclose the literature survey of the state of the matter in pediatrics.

The article highlights diet optimization in pregnant women and nursing mothers with «Femilac» (zao company «Nutritec», Russia) and «Lactamil» (zao company «Nutritec», Russia) providing composition, indications for use and clinical efficacy assessment. The article reviews chemical composition and clinical trial results of the pediatric milk formulas «Nutrilac 0–6», «Nutrilac 6–12», «Nutrilac 0–12», «Nutrilac Bifi» and «Nutrilac KM» developed for mixed and formula feeding of babies and infants under 1 year.

The lecture presents current opinions on the problem of topical treatment of atopic dermatitis in children discussing different topical antiainflamatory drugs with and without corticosteroids. pimecrolimus 1% cream (elidel, novartis pharma, Germany) is specially emphasized among the latter. Pimecrolimus is shown to provide symptom relief and control in mild and moderate cases of atopic dermatitis, so it could become essential in preventing exacerbations and elongation of remission periods of the disease.

EXCHANGE OF EXPERIENCE
The center for diagnostic, treatment, rehabilitation and social adaptation of patients with cystic fibrosis was established in pulmonology department of the children's clinical hospital № 3 of Novosibirsk in 1994. Since 1995 the database of the center has registered 23 patients. the center has carried out an open prospective study of clinical efficacy and safety of dornase alfa (pulmozyme, «F. hoffmannala roche ltd.», basel) in 10 patients with cystic fibrosis. The patients have received mucolytic therapy with dornase alfa for 2 years (2003–2005). They have been administered a daily inhalation of 2.5 ml of the solution (pulmozyme inhalation solution) containing 2.5 of dornaza alfa. The study has shown an improvement of pulmonary function and elevation of oxygen saturation. The number of respiratory infections have dropped by 33%, the administration of intravenous antibiotics has shortened by 25%, the nutrition status has trended towards improvement. regardless of it's high price, dornasa alfa has proven its' benefits in the treatment of cystic fibrosis in patients of any age and disease severity.

The report provides the assessment of preterm neonates with perinatal infections (intrauterine and nosocomial) with concomitant cerebral hypoxiaaischemia and respiratory disorders and their humoral immune status. The adding of intravenous immunoglobulins to the complex treatment of perinantal infections in preterm neonates has led to significant decrease of mortality rate and shortened the time spent in resuscitation and intensive care department. intravenous immunoglobulins contribute to activation of humoral immunity and nonaspecific resistance factors.

Voiding dysfunction is a common problem in pediatrics. Hyperactive urinary bladder is the most frequent cause of voiding dysfunction. Macholinolytic drugs are the first choice treatment for urinary bladder hyperactivity. The «urination pathology» center has investigated the efficacy of an macholinolytic, oxybutynin. The study included 25 children (aged 6–14) with different causes of voiding dysfunction and bladder hyperactivity. Oxybutynin was administered with a dosage of 5 mg twice a day, the follow up period was 8 weeks. The control evaluation of urodynamics (voiding rhythm, uroflowmetry, residual urine volume) was performed at 4–8 weeks. Symptoms of bladder hyperactivity have been eliminated in 50% of patients, the bladder capacity has increased in 70% of cases, the frequency of night and day incontinence has diminished. The report demonstrates that oxybutynin has been most effective in patients with urinary bladder volume between 80 and 100 ml. the side effects, as dryness of mucous membranes and voiding difficulties, have been rarely observed. These side effects could be avoided by individual dose titration.

SOCIAL PEDIATRICS AND HEALTH CARE
The article highlights the importance of scientific approach in development of state policy in pediatrics healthcare as an integral part of policy guarding the benefits of children in current socio economic conditions in Russia. The author gives definition to the terms as «child's health» and «state policy in pediatric healthcare» and proves the methodology and practice of the current social policy in solving problems of children to be untenable. The author suggests to revise our perception of children — from the «social groups» to a «growth zone of society».

The article provides the analysis of statistic reports from the agencies providing stray children care in comparison with 2570 questionnaire reports collected from stray children and medical evaluation of those who were hospitalized. The article high lights medical and social features of stray children health, estimates the value of social maladaptaion risk factors and offers a model of multilevel system of rehabilitation for stray children.

The efficacy of aqua Maris as a mean for prophylaxis of influenza and other viral upper respiratory tract infections (Vuri) in children between 6 and 10 years (primary school age). It has established that elimination therapy targeted to reduce viral and bacterial colonization of upper respiratory tract is a promising trend in vuri prophylaxis in large groups of children.
