Abstract
The quality of life in 120 children and adolescents with different types of tuberculosis, treated in Astrakhan Regional Children’s sanatorium for the Treatment of Patients with All Types of Tuberculosis was evaluated with PedsQL (value 4.0) questionnaire. Authors estimated an initial level of a quality of life before the beginning of hospital treatment and right after its finishing. They analyzed changes in quality of life in children of different age, boys and girls separately, and according to the subjective estimation of parents (tutors) and children themselves, and also according to the separated scales of functioning.

Abstract
The article presents the analysis of quality of life of adolescents from Udmurtiya, undergoing prophylaxis of tuberculosis. Author used PedsQL 4.0 questionnaire. 50 adolescents and equal quantity of parents took part in the study. It was shown that the prophylaxis of tuberculosis was performed in adolescents from families with low education level and with low earnings of parents. They have statistically significantly low level of quality of life compared to healty children.

Abstract
Heterogeneity of juvenile arthritis conditions the difficulties in estimation of its veritable prevalence among children and in social significance of the disease, leading to early arising of patients’ disability. The work analyzes age, demographic and clinical peculiarities of juvenile rheumatoid arthritis in children and adolescents (n = 117), living in Rostov region. It was shown that in Rostov-na-Donu and other cities of Rostov region children had II grade of activity of disease, I and II anatomic grade, II functional class, slowly progressing flow of disease. In rural areas of Rostov region the patients with III grade of activity (67%) were prevalent, and III–IV anatomic class was seen frequently (56%), the degree of disability developing was higher. The clinical course of a disease has slowly progressing character in 79% of patients. The activity of disease, anatomic class and degree of disability in girls are higher than in boys. The articular syndrome is more common in girls and swelling of joints — in boys. The activity of articular syndrome increases in older patients.

Abstract
The article presents the results of the observation of 200 children under the age 18–42 months (64 healthy children and 136 patients with allergic symptoms, residual lesions of CNS, frequently ailing children, and having reactions of previous vaccination in medical history) who were re-vaccinated with vaccine Pentaxim. It was shown that 77% of children had asymptomatic post-vaccinal period. Only 1,5% of patients showed severe reactions with fever > 38,6_С. Local reactions (not over 3–5 cm) developed in 25,5% of children with allergy, lesions of CNS and frequently ailing children, their rate was significantly higher than in healthy children (7,8%). No one had post-vaccinal complications. An estimation of reactogenicity of vaccine proved its safety and suitability of administration as first re-vaccination against pertussis, diphtheria, tetanus, poliomyelitis and one-time vaccination against Haemophilus influenzae type b in children after 1 year old in different state of health.

Abstract
Functional sizes of school furniture are established on the basis of the analysis of results of anthropometric studies and are regulated by State standards. The article presents the results of anthropometric measures of 923 schoolchildren from 1–4 forms. 6 indices, which determinate functional sizes of school furniture: height of popliteal space, height of elbow above the sitting, height of shoulder above the sitting, a distance between back and popliteal angle, the most width of pelvis, length of a body were measured. Changes of body’s proportions in modern younger schoolchildren were proved: increasing of the length of upper and lower extremities, decreasing of body length. These facts lead to the need of reconsideration of normative sizes of school furniture and rules on its choice for the organization of optimal place of work of younger schoolchild.

Abstract
The study of effectiveness of different methods of microbiological disorders correction in children after 3 years old with candidal dysbacteriosis are presented in this article. The study compared probiotical sour milk-made stuff («Actimel») and sour milk-made stuff, not fortified with probiotical cultures («Rastishka» and traditional kefir). It was shown that an inclusion of probiotical sour milkmade stuff in diet of children with candidal dysbacteriosis results in normalization of lacto- and bifidobacteria level and decreasing of Candida level.

Abstract
The results of an observation of 288 children under the age 3–20 months old (46 healthy infants and 142 patients with allergic diseases, residual lesions of CNS, frequently ailing ones and infants with other pathologies), vaccinated and re-vaccinated with Hiberix and Infanrix in one syringe were analyzed. High safety of such method of injection allowed decreasing of injection load during the vaccination against hemophilic infection type b in infants. Common moderate reaction was detected in only one child (0,5%). Topical reactions were registered in 5,0% of vaccinated patients.

Abstract
The article presents a review of literature data, dedicated to problems of secondary ineffectiveness of infliximab in patients with rheumatoid arthritis. An analysis of available data showed that the main reason of resistance to the TNF α inhibitors is a synthesis of antibodies to the drug. Risk factors of antibodies production are absence of induction phase in treatment, prolongation of interval between infusions longer than 8 weeks, infliximab monotherapy. Treatment with chimerical antibodies to TNFα according an instruction: infusion on 0, 2 and 6 weeks and further not longer than in 8 weeks with necessary combinations with methotrexate in standard dose, — may decrease the risk of development of primary and secondary ineffectiveness of infliximab.

Abstract
The review analyzes the modern opinion on the problem of functional disorders of gastrointestinal tract, which are peculiar to infants. The peculiarities of nutrition and intestinal microbiocenosis in healthy infants are discussed. Authors observe the ways of its correction and propose the principles of a choice of medications, having proven clinical effectiveness in children on early stages of postnatal development. The advantages of natural smectite dioctaedric (Smecta) in prophylaxis and treatment of functional disorders of gastrointestinal system, acute and chronic diseases in children are presented.

Abstract
The series of lectures present the data on typical syndromes of organism’s thermal balance alterations: hyperthermic and hypothermic states. Present lecture analyzes an etiology (including the data on initial and secondary pyrogens), key components of mechanism of development and manifestation, information on temperature curves, changes of metabolism and organs’ function, on adaptive and pathogenic effects of fever reactions. Authors give a data on mechanisms of fever and principles and methods of its etiotropic, pathogenetic and symptomatic treatment.

Abstract
Behcet’s disease is a systemic vasculitis with unknown etiology. It is characterized by recurrent erosive and ulcer lesions of mucoustunic of mouth and genitalia, frequent lesions of eyes, joints, gastrointestinal tract and central nervous system. Behcet’s disease arises predominantly in young men in the age 20–40 years old; the clinical signs of this disease are forming in children up to the 7–13 years old. Approximately 2–3% of patients with Behcet’s disease are children under 16 years old. The diagnostic criteria of disease, presented in 1990 with International Group of Behcet’s Disease are presented. The comparison of clinical signs of a disease showed bigger frequency of vascular pathology in adults; the rate of HLA-B5 antigen is considerable in children and adults (58,5% and 62,8% accordingly) but an aggregation of disease in families of children with Behcet’s disease is higher (12% vs. 1% in adults). Authors presented EULAR recommendations (2008) on the treatment of Behcet’s disease. The peculiarities of treatment of Behcet’s disease are described.

Abstract
Chronic constipations are actual problem in children’s gastroenterology. The development of constipation in children is conditioned by anatomical and functional peculiarities of organs of gastrointestinal tract, including large intestine. Besides, the pathogenetic role of nutrition disorders and psycho emotional features of child are significant factors. Author presents the results of his own experience of administration of osmotic laxative medication macrogol 400 (Forlax). It was shown that treatment of children with constipation with course of this drug is effective, well-tolerated and safe; it provides a normalization of stool and an improvement of patients’ feeling.

Abstract
Technological progress saved human from hard physical work and big expenditures of energy. It was resulted in need to decrease the volume of consumed food. But need in vitamins remained on the former level. Maximal volume of vitamins, which organism use, is conditioned on the level of genes. The reasons, allowing a development of state of hypovitaminosis especially in schoolchildren, remain. Increased metabolism, induced by the growth of organism, frequently inevitable stress, sometimes unbalanced nutrition, hard intellectual loads, etc., can lead to the development of the states, which need increased doses of vitamins. Observations during last 20 years showed that there is no almost any child, whose provision with vitamins con be estimated as optimal. In most children (up to 70%) the combined deficiency of three and more vitamins is detected, independently of their age, time of year and place of living. Pediatricians use vitamins not only for the maintenance of homeostasis itself, but for the initiation of all program of child’s development. Schoolchild’s organism needs additional volume of vitamins all the time, and they should be administrated during prolonged periods of time, independently of time of the year and adequacy of diet. Timely administration of vitamins and minerals complexes provides significant prophylactic of hypovitaminosis.

Abstract
The article presents a case report of severe clinical course of systemic type of juvenile rheumatoid arthritis refractory to the immunosuppressive treatment. The disease was characterized with such extra-articular symptoms as fever, lymphadenopathia, hepatosplenomegaly, polyserositis. Generalized joints lesion and high laboratory signs of disease’s activity presented. Significant hormone-dependence and hormone-resistance on the ground of corticosteroids developed. Inclusion of rituximab to the scheme of treatment stopped systemic symptoms of a disease, inflammatory lesions in joints, and normalized laboratory rates of activity. Beneficial effect of treatment allowed lowering of peroral corticosteroids’ dose. Obtained results testify to high effectiveness of rituximab. This medication can be the drug of choice in a treatment of children with systemic type of juvenile rheumatoid arthritis, refractory to immunosuppressive therapy and corticosteroids.

Abstract
The share of acute respiratory infections is more than 90% of all diseases of upper airways. It mostly related with insufficiency of immunological defense, which elements are localized in different organs, including mucous tunic of upper airways. The article presents the data on high effectiveness of complex treatment of frequently ailing children with respiratory infection, including topical immunomodulator IRS 19 (bacterial lysates mixture). Its administration combined with antibiotics and antiviral medications allowed achieving more rapid fauces sanation. In particular, significant decreasing of pathogenic and conditionally pathogenic micro flora (neumococcus, neisseria, and Staphylococcus aureus) was marked in 3 months after the beginning of treatment.

Abstract
90 frequently ailing children (FAC) under the age 3–7 years old with recurrent bronchitis were observed. In 53 (59%) of them a colonization of mucous tunica with Candida and Staphylococcus aureus was revealed. The complex treatment of bronchitis in 33 children included fusafungine (Bioparox), having high activity against S. aureus and Candida (4 nasal and 4 oral inhalations 4 times daily during 7–10 days). The control group contained 20 children, receiving traditional treatment. It was shown that complex treatment with fusafungine is effective in 79% of cases. The duration of exacerbation was decreased 1,8 times lower and the remission was prolonged two times longer on the basis of negative results of cultural mycological and bacteriological tests. Described complex treatment with fusafungine can be widely used in FAC with recurrent bronchitis, associated Candida and S. aureus.

Abstract
The article describes an efficacy of interferon alpha-2a (Roferon-A) in 69 children with chronic viral hepatitis C. The frequency of forming of initial and prolonged remission and the role of such factors as viral genotype, the level of viral load, terms of elimination of virus, the evidence of histological changes in liver’s parenchyma in prognosis of remission and late relapse development was shown. Authors estimated the safety of the drug.

Abstract
Authors studied an effectiveness of antibiotic rifaximin in complex treatment of Helicobacter pylori infection in children. They observed 41 children in the age 12–17 years old (23 girls and 18 boys). The diagnosis of pyloric helicobacteriosis proved by histology and bacterioscopy with Gimze stain combined with urease test. All children were treated according to the study program with the use of medications: bismuth (De-Nol) — 1 tablet 3 times daily (14 days) + rabeprasole (Pariet) 1 mg/kg once daily (14 days) + rifaximin (Alfa Normix) 400 mg twice daily (10 days) + amoxicillin 50 mg/kg twice daily (10 days). The control of eradication of H. pylori was performed after 4–6 weeks after the end of a treatment with a respiratory test. Eradication was achieved in 36 (87,8%) of children. All patients were adherent to the treatment, and there were no severe adverse reactions during the study.

Abstract
Chronic tonsillitis is the infectious and allergic disease; the main clinical symptom of this disease is chronic inflammation of palatine tonsils. The prevalence of chronic tonsillitis is up to 10% among adults and 15% — among children. Undoubtedly, the prevention of exacerbations of chronic tonsillitis is difficult task. Its significant component is immunocorrection with elements of vaccinal process, because tonsils themselves are the part of forming immune system of a child. Imudon can be recommended for the inclusion to the yearly prophylactic treatment courses in patients with chronic tonsillitis, hypertrophy of palatine tonsils and recurrent tonsillopharyngitis.

Abstract
The article is dedicated to the one of the most frequent clinical manifestations of papillomavirus infections – the anogenital condylomas. It is known fact, that viral papilloma infection is the most prevalent sexually transmitted infections. The author described clinical signs of genital condylomas and proved the urgency of this problem. Taking into account low effectiveness of treatment, the unique opportunity of initial specific prophylaxis of a disease with vaccination is observed.

Abstract

Abstract
The article presents the main principles of optimal nutrition organization in infants. According to the «National Program of Nutrition of Infants Optimization in Russian Federation» (2009), accepted on ХVI Congress of Pediatrists of Russia, the terms of feeding up are revised. Abstract
Administration of etiotropic medications, blocking a penetration of virus into the cell and/or suppress its replication, is advisable additionally to the vaccination as main strategy against acute respiratory viral infection (ARVI) and influenza in children. As etiological diagnostics of ARVI and influenza demands time and sometimes it can’t be provided, the antiviral treatment may be started if there are indications for its administration without identification of pathogen, following medical history and clinical picture of the disease. One of the main prevalent medications with etiotropic activity is rimantadine. New polymeric medication for children based on rimantadine is Orvirem. It is complex of rimantadine, fixed to sodium alginate, amplifying antitoxic activity of a medication and providing its improved tolerance. Such pharmacological formula provides gradual entrance of rimantadine into the blood, its prolonged circulation in organism, constant concentration in blood and decreasing of toxic effect of the drug. Due to these properties, the medication can be used for the prophylaxis and treatment of influenza and ARVI in children older than 1 year old. The medication combines as direct antiviral activity, as immunomodulating effect, resulting in restoration of cell immunity. Clinical studies showed drug’s effectiveness and safety in patients with influenza type A and B, with adenoviral, respiratory-syncytial viral and mycoplasmal infections. Prophylactic effect of the medication in inevitable contact with patients with ARVI, including children’s group was also proved.

Abstract
Authors give characteristics of pathogenetic and clinical correlations between influenza and pneumococcal infection. The results of analysis of data from international studies, confirming the role of pneumococcal infection as main reason of mortality during the influenza pandemiс of 1918 year are presented. The role of vaccination against pneumococcal infection with the use of pneumococcal conjugated 7-valent vaccine in children under 5 years old and polysaccharide 23-valent vaccine in adults undergoing high risk factors, for the purpose of influenza complications including new high-pathogenic influenza A/H1N1 California, is shown.

Abstract
Inflammatory demyelinating diseases in children and adolescents are sufficient problem of modern medicine. Its topicality is conditioned by as traditionally high prevalence of acute demyelinating conditions (acute disseminated encephalomyelitis) as increase of morbidity with chronic demyelinating forms of pathology (disseminated sclerosis) in children. The absence of clear diagnostic algorithm presents difficulties for the determination of tactics of treatment and prognosis on early stages of their development, e.g. at the time of maximal effectiveness of immunomodulatory medications. Besides, the list of medications for the prolonged immunomodulatory treatment of demyelinating diseases, registered in Russia, includes such drugs as interferon beta 1a for the subcutaneous injection, officially annotated for the use from 12-year old age.

Abstract

Abstract
The article presents a case report of severe juvenile rheumatoid arthritis with uveitis, refractory to treatment with classic immuno-suppressants and chimeric monoclonal antibodies to the tumor necrotizing factor ?. Successful administration of biological agent adalimumab was described: morning joints stiffness and exudative lesions in affected joints was completely stopped up to the 4th week of treatment, the range of motions was increased significantly. Laboratory indices of disease activity (ESR, C-reactive protein in plasma) were normalized to 6th week. Treatment with adalimumab induced the development of remission of subacute bilateral uveitis.
