Abstract
Scientific center of children's health, Russian academy of medical sciences the presented article is dedicated to the urgent issue — formation of the legal grounds of the state policy in children's healthcare protection. Abstract
The article is dedicated to the issues of the level and structure of morbidity, invalidity of the orphans. The author has analyzed the reasons for the orphans' deaths in detail. She estimated the level and quality if the medical care for the orphans. There are some organization approaches to solve the existing issues of complex medical and social care for the children of this category.

Abstract
The article gives a population estimate of the social and biological hazards of noncinfective diseases among children, living under extreme conditions of the far north. The authors show the high spread level of the most indicated factors. The researchers have identified almost all the factors of the nonc infective risk among the children cnortherners, who were born as a result of the pathologically run pregnancy. It is supposed to use the obtained data during the development and realization of the complex study and sanitary programs for the children, living in the regions of the far north.

Abstract
The preventive efficiency of various kinds of feedings in children of the first year of life with high risk of an allergic pathology is studied. Under observation there were 87 children from group of high risk of an allergic pathology (from birth up to 18 months). The results of this study have confirmed the importance of breast feeding for prophylaxis of an allergic pathology in children with high risk of atopy. If it is impossible, the transclation of the child on artificial feeding by an dairy admixture based on a partial hydrolysate of serumal protein, which procvides physiological needs of a growing organism, assists physiological dynamics of parameters of weight and height, allows to increase frequency of early manifestation of atopy and prevents a sensibilization to proteins of the cow milk.

Abstract
3120 patients with recurrent paroxysmal abdominal pain were registered in the clinic in the past 3 years. 138 patients aged 7–15 didn't reveal any pathologic changes in the abdominal and pelvic cavities. Clinically, all patients demonstrated paroxysmal attacks of local abdominal pain lasting for a time between 6 months and 4 years. Clinical examination and ultrasound scans gave evidence to link the abdominal pain (syndrome) with changes in appendix as the only cause of it. All 138 patients underwent laparoscopy with total abdominal and pelvic cavities revision. No other pathology but significant macroscopic changes of appendix has been found in all cases, so it has been appraised as a macroscopic sign of chronic inflammation. Morphologic studies of excised appendices has demonstrated the signs of chronic inflammation in all cases.

Abstract
The work studies the efficiency of the specialized milk formula inclusion into the diet to prevent and correct the calcium deficiency among infants (aged between 1 and 3 years old). The authors discovered the positive impact of the milk formula on the values of the phosphoric and calcic exchange: the level of the ionized calcium, daily excretion of calcium with urine, osteocalcin. They identified the significant effect of the diet therapy for the prevention of the pathologic symptoms on the part of the musculoskeletal system (bearing disorder, reduction of the muscular tonus and etc.).

Abstract
The researchers examined how well the children aged between 3–5 years old were provided with с, В1, В2, В6 and РР vitamins both before and after 3 months of taking in two forms of the polyvitaminic complex (syrup and pastilles). The examination was performed on the basis of the renal excretion of the ascorbic acid, thiamine, riboflavin, 4cpiridoxyl acid and 1cmetylnicotinamide. The polyvitaminic complex intake led to increase of excretion of all the examined vitamins apart from the ascorbic acid, warned about the deterioration of provision with с and в6 vitamins and some what improved children's provision with В1, В2 and РР vitamins. However, the intake of this complex in the applied doses didn't ensure complete normalization of the vitamin status indices among all the examined children, which indicates the necessity to apply it in bigger doses, as the manufacturing firm recommends to.

Abstract
The article deals with the concept of food programming according to which the type of nourishment of an infant programs peculiarities of his metabolism all along his entire future life, and as a consequence, predisposition to certain diseases and characteristics of their run. The work gives an estimate of the hazards and consequences of improper infant feeding and explains the advantages of the breast feeding. The authors present modern information on preparation of baby milk formulas.

Abstract
The given article presents data on the findings of the two large clinical studies performed in Sweden, whose purpose was to examine the degree of the iron uptake from the breast milk substitutes of the new generation by the children (reduced protein contents, enrichment by C lactalbumin and etc.), depending on the level of the enrichment and form of iron, its impact on the absorbality of other microelements. The researchers received the data, which became the basis for the new commendations for the iron contents in the breast milk substitutes that was reduced to 2c4 mg/l in Europe for the infants.

Abstract
The article is dedicated to one of the most urgent issues of the pediatrics — rational antibacterial therapy of the community acquired pneumonia among infants. The authors highlighted the age peculiarities of aetiology and basic criteria for diagnostics, clinical peculiarities and principles of empirical selection of the starting etiotropic therapy for pneumonia among children. They characterized the antibacterial medications allowed for the treatment of pneumonia among children, as well as their regimes of dosage and injection methods.

Abstract
The priority of the breast feeding is undoubted, although the significant part of the infants receives the milk formulas. The constant expansion of the assortment of the adapted and specialized baby formulas requires additional knowledge of the pediatrician. The article anlyzes the peculiarities of the clinical application of the formulas with nucleotides and oligosaccharides; antireflux formulas; hydrolysates based on the serum protein and casein; products for the premature children.

Abstract
International conventional views on a problem of pubertal uterine bleeding are elucidated in the article. Clinical peculiarities and risk factors, diagnostic methods and therapy of pubertal uterine bleeding in girls is thought out considering multiple factors of pathogenesis and concomitant diseases. Revealed principles of interdisciplinary treatment of girls with uterine bleeding by pediatricians and gynecologists for children and adolescents are designed to facilitate early of premorbid health status, features of reproductive organs development and genital organs state. These steps would allow to hamper pathology progression and result in better levels of reproductive health in mothers in the future.

Abstract
Within recent years, a special attention has been paid to the nonspecific prevention of the acute respiratory disease related to the increase of the activity of the natural mechanisms for antiviral protection. The application of the liposomal forms of the different medications contributes to the directed transportation of the biodegraded protein substances. A special interest is aroused by the opportunity to orally apply protein medications, as their injection forms quickly degrade in the stomach. New Russian liposomal recombinant alphac2b interferon has antiviral, immuno-modulating and interferonogenic activity. The work demonstrates experience of the oral application form of the liposomal medication of recombinant alphac2b interferon — reaferoncesclipint for the extra prevention of flu and other acute respiratory infections among children. The application of this medication to prevent flu and acute respiratory infections in the dose of 250,000 Ме twice a week for the 4 weeks' period proved to be efficient within the group of the preschool children (aged between 7–10 years old) and manifested itself in the reduction of the flu and acute respiratory infections recurrence.

Abstract
Pneumococci are spread everywhere and they are very often a component of the microflora of the upper respiratory tracts. The level of the pneumococcus carriage is correlated with age. Among children the highest frequency is observed at the age of 4,5 years (up to 90% of cases), among adults it is 5–10%. According to international and Russian data, pneumococcal infection causes up to 76% of all the aetiologically deciphered cases of community cacquired pneumonia among adults and up to 94% (aggravated cases) among children. The most frequent clinical forms of pneumococcal infection among children are acute otitis media (over 30%), pneumonia and meningitis (about 5–20% of all purulent bacterial meningitis), among adults — meningitis and sepsis. In 1998, in Russia was registered the first and still the only vaccine for the prevention of pneumococcal infection — Pneumo 23 (Sanofi Pasteur). The vaccine consists of 23 antic gens of the most dangerous pneumococcus serotypes and is used for the prevention of all the forms of pneumococcal infection. The composition of Pneumo 23 corresponds to 85% of pneumococcus serotypes circulating across Europe and to 90% serotypes resistant to penicillin. According to Russian data Pneumo 23 consists of about 80% of pneumococcus serotypes isolated in healthy carriers and ill with acute respiratory diseases and of 92% of serotypes in those suffering from acute bronchitis and pneumonia. The results of the clinical studies allow us to recommend the use of the given vaccine in a complex therapy of children, suffering from latent TB infection, often recurrent episodes of bronchopulmonary pathologies, ENT diseases, bronchial asthma and other chronic diseases.

Abstract
The article deals with the research findings of the application efficiency of bacterial immunocorrector IRS 19 (Solvay Pharam, France) among children, suffering from chronic adenoiditis and hypertroc PHY adenoid vegetations of II–III degrees. The author ascertains that treatment by bacterial immunocorrector has led to quicker and more qualitative normalization of the lymphoid tissue volumes, than nasal pharynx irrigation by salines. The application of bacterial immunocorrector during chronic adenoiditis has contributed to normalization of nasopharyngeal secretion boicenosis and considerably decreased the degree of pathogenic flora dissemination. Another proof for that is reduction of the number of adenoiditis exacerbation by 2 times, duration of all the exacerbations by 3 times, as well as reduction of the disease duration. The research findings allow advising introduction of the bacterial immunocorrector into the planned seasonal programs of the annual treatment (no less than 2–3 times a year) of children with chronic adenoiditis.

Abstract
For the purpose of this work, the authors studied the efficiency of the dutch instant goat's milk «Amaltea» among preschool and school children with aggravated forms of atopic dermatitis and mycotic infection. Under surveillance, there were 66 children aged between 3 and 18 years old with atopic dermatitis aggravated by the fungal infection. 36 of them received the dutch wholesome goat's milk «Amaltea» within the hypoallergenic diet. The test group was composed of 30 children, getting the milk free diet. The use of the dutch instant goat's milk «Amaltea» within the ration of the infants with atopic dermatitis aggravated by the fungal infection leads both to the short cterm positive results — clinical remission on the 12–20th day from the beginning of the therapy and to the long cterm positive effects — prolongation of the remission period by 3 times, reduction of the recurrences by 2.8 times against the decrease of the general IgE contents within the blood serum and allergenic specific IgE antibodies towards the proteins of the cow milk and casein. Thus, the replacement of the cow milk based products for the wholesome goat's milk «Amaltea» within the ration of the patients allows optimizing the diet therapy among preschool and school children, suffering from atopic dermatitis.

Abstract
The search for nootropic medications, which don't aggravate the implications of the hyperactivity disorder and sleep disturbances, is an urgent issue in the children's neurology. For the purpose of pantohamium sirupus appraisal, the researchers examined 20 patient aged between 3 years old and 4 years 11 months old, suffering from epilepsy with cognitive disorders and attention deficit and hyperactivity disorder. All the patients underwent interviewing with the composite questionnaire, oddball tasks, mechanical memory tests (memorizing 3–4 words), dynamic memory tests (repeating 2 groups of words), colour test (for the children of 4 years old), as well as electroencephalography and by indications clinical and biochemical examination. After the course of panto hamium sirupus treatment the researchers discovered the general improvement of attention, mechanical and dynamic memory, anxiety reduction and compensation boost based on the colour test conducted among the children, suffering from epilepsy with attention deficit and hyperactivity disorder. Most patients showed the improvement indices of the EEG background rhythm structure and normalization of background zone differences. The analysis of the obtained results allowed the researchers to conclude that pantohamium sirupus was safe and efficient in the complex therapy among the children, suffering from epilepsy with cognitive disorders and attention deficit and hyperactivity disorder.

Abstract
The article deals with the findings of the endoscopic examination and treatment of 80 children, suffering from rheumatic diseases. According to the esophago gastroduodenoscopy data, all the patients turned up to have inflammatory changes in the upper gastrointestinal tract of various degrees of manifestation. Moreover, only 36% of patients complained about pain in the stomach. The application of esomeprazole in the complex therapy contributed to the epithelization of erosive and ulcerous defects and dyspepsia syndrome relief among the majority of the patients. The effects of the complex therapy with esomeprazole were appraised as good among 90% of children and excellent among 9% of patients.

Abstract
Due to immaturity of the digestive system of the premature children under artificial feeding there may often be effects of gastrointestinal tract dysfunctions, which disturb postnatal adaptation among these newborns. The purpose of this research was to analyze a number of indices, characterizing the dynamics of the body weight and functional status of the gastrointestinal tract among the premature children, who received modern active digestive ferment at different periods of time. The authors presented the findings of the retrospective analysis of the case history among 29 premature children, who were held in the department of the premature children of the perinatal center. Children received pancreatine ferment (Creon 10000, Solvay Pharma, Germany). The ferment was introduced in the dosage of 150mg/day. This dose was orally introduced by minimicrospheres during every feeding and in some cases through the nasogastric probe. The ferment intake started in the early neonatal period among 13 children and in the late neonatal period among 16 children. During the course of the ferment therapy the frequency of the symptoms of the gastrointestinal tract dysfunctions drastically reduced. When the course of the ferment therapy was prescribed at an early age, the body weight increase tended to higher indices among premature children during the first month of their lives.

Abstract
The dietary guidelines for americans contains recommendations on diets and healthy lifestyles but it does not deal with specificities of the diet and feeding breast and small infants (to 2 years). However, parents and carecgivers need practical recommendations on feeding infants and toddlers that are necessary to ensure a normal growth and to create the basis for forming healthy food habits, which apart from other benefits might help prevent the development of obesity and other diseases in both childhood and adulthood. The feeding guidelines for infants and toddlers is aimed at providing parents and carecgivers with practical information on feeding breast and small infants. It is intendced for adding and broadening recommendations worked out by other expert groups and organizations. The article provides a substantiation of the guidelines. The methodology, results and a complete reference list used to work out the recommendations are presented in another document.

Abstract
The article is dedicated to the study of the peculiarities of atopic dermatitis run from the viewpoint of allergistcimmunologist and gastroenterologist. The authors give an analysis of the reasons for atopic dermatitis development conditioned by the food allergy and define the place and meaning of the digestive apparatus function within the mechanisms of the disease development. The authors dwell in detail on the state of the intestinal tract mucosa and peculiarities of its lesion during atopic dermatitis. They give the schemes of the combined treatment for atopic dermatitis aimed at recovery of the affected small bowel mucosa and recovery of its protective properties with the help of cytomucoprotective adsorbing agents. They also present the findings of their own clinical experience of treatment of children, suffering from atopic dermatitis and having various lesions of the digestive apparatus.
