Abstract
The basic trends of development of social pediatrics are described in this article. The main attention was given to investigations, favored to the reduction of loss of children's health related to morbidity and disablement, which can be eliminated with appropriate medical care and prophylactic measures, and mortality of children. This measures have to be based on proper public health politics.

Abstract
The article focuses on the analysis of modern medical social aspects of health forming in children in junior school. High level of risk factors of diseases, concerned with mode of living, was discovered among children in junior school. Such factors as hypodynamia, sleep deficiency, a prevalence of static component over dynamic one, insufficient consumption of basic food stuffs, violation of dietary habits and parent's smoking are responsible for decrease of adaptive resources of child's organism and disorders in a mechanisms of vegetative regulation. These factors determine a high risk of vegetative dysfunction, underlying deviation in functioning of different organs and systems. Authors offer to use obtained data in development of complex prophylactic programs for children in junior school.

Abstract
Poverty is the actual problem in modern Russia. Complex clinical, laboratory and instrumental diagnostics and social and hygienic research in 108 children at the age of 5–7 years from poor families (basic group) was performed. 108 children from families with mean income, comparable to the children from basic group by age and sex, formed control group. The peculiarities of state of health in children were discovered using appeal ability and advanced examination and physical growth and development data, school readiness was estimated. Authors made a conclusion of health group.

Abstract
Recently discovered phenomenon of compensatory mechanism of absence of legislative durable national policy in health protection of children is described in this article. This mechanism was developed as a result of evolution of intersect oral connection for the sake of children's health protection. «National family policy», «national policy for the sake of children», «national policy in the field of circulation of narcotic, psychotropic substances and in the field of counteraction to its illegal use», «national policy of Russian federation in the field of education», «national policy in the field of labor relations» perform functions of such compensatory mechanism. Fact of regulation of social relations in the sphere of children's health protection with means of different active directions of national policy is evidence of legal assignment of a number of problems in children's health protection as real subject of policy. The negative feature of this problem is incomplete and fragmentary regulation of relationships in the field of children's health protection. This is the reason, which maintains a status of children's health protection service as passive dirigible object but not an active regulative factor.

Abstract
Frequency, structure and consequence of different risk factors of intra ventricular bleeding in newborns are presented in this article. This data was obtained from results of statistical and correlation analysis of information from delivery cards of women with risk factors, resulting in fetal hypoxia and lesion of CNS in newborn. These risk factors were discovered in women at the time of registration in antenatal clinic because of pregnancy and at the period of gestational process. prediction of risk rate of pathology onset was calculated with method of normalized intense indices.

Abstract
An objective of this trial was evaluation of effect of treatment with blocker of tumor necrosis factor infliximab on dynamics of functional class and radiological changes of bone and cartilaginous tissue of joints in children with different clinical types of juvenile arthritis (JA). Infliximab was used according to standard scheme on 0, 2, 6 and then every 8 week by intravenous infusion. Mean single dose of infliximab was 6,8 ± 2,3 mg/kg. All children were treated with immunosuppressive agents (ciclosporin in 6% of patients, methotrexate in 60%, ciclosporin combined with methotrexate — 20%, ciclosporin with leflunomide — 5%, leflunomide with methotreate — 4%) combined with anti cytokine treatment. 7 children were treated with oral glucocorticoids in mean dose 0,37 mg/kg daily. disorders in joint function and structural changes of joints of different severity (radiological sign) were discovered in all patients before starting of treatment with infliximab. Duration of observation was 6 week — 2 years. Authors made a conclusion that treatment with infliximab prevents progressing of bone and cartilaginous destruction in all types of AJ, not depending on intensity of its clinical activity. This medication provides for full functional recovery of joints and elimination of features of disablement in patients with good clinical activity.

Abstract
100 mothers and children in couple were observed in this trial. All children had atopic dermatitis, developed against breast feeding. Analysis of immunologic characteristics of breast milk in 53 women showed a presence of allergencpecific Ide and Igg antybodies and common Ige in it. The level of those characteristics was lower if mothers were consistent with diet with goat's milk «amaltea».

Abstract
He structure of microbial inflammatory diseases of urinary tract in children of early age is described in this article. Clinical course of infectious diseases of urinary tract in first 3 years of life is characterized by absence of specific clinical features. Increase of activity of kallikrein and angiotensin converting ferment, and decrease of callikreinogen and α-2-macroglobulin testify to infectious process in kidney in children of early age. Increase of excretion of albumin with urine in patients with nephropathy is an evidence of glandular lesion at microbial inflammatory process in renal tissue.

Abstract
Etiological factors of congenital malformations of spinal column and spinal cord were described on the basis of literature analysis and results of proper observations. Measures of early diagnosis and prophylaxis of abnormalities, which can decrease rate of children's disablement and mortality, were indicated.

Abstract
One of modern actual problems of prophylactic pediatrics is vitamin and mineral provision of schoolchildren. The information about prevalence, etiology, clinical manifestation of vitamin starvation in children was described. The measures of prophylaxis of hypovitaminosis with use of food substances enriched with vitamins and microelements, and with polyvitamin and mineral complexes, fortified drinks, premixes were reviewed. Recommended daily consumption rate of main vitamins and composition of modern vitamin and mineral complexes was indicated.

Abstract
15% of children in the world have allergic diseases (who data). The rate of allergic diseases in citizens of megapolices in Russia is 30–60%. The number of visits to the doctors increased on 18% in last 3 years. Generalized allergic reactions have leading place: anaphylactic shock and toxico-allergic dermatitis (steven johnson and lyell's syndromes). These diseases are characterized as pathology with fulminant course and frequently negative outcome. Literature data about this problem was analyzed in this article. The results of own researches in the field of urgent medical care on pre admission stage in children with generalized allergic reactions were presented.

Abstract
The structure of morbidity of respiratory system diseases (RSD) in children, living in Saint Petersburg, is described in this article; also the role of prolonged types of RSD is indicated. Main etiologic factors of prolonged RSD are presented, and short clinical characteristics of prolonged RSD are given. Main methods of treatment of negative course of bronchopulmonary diseases are indicated. The data of 35=year clinical experience of use of n-acetylcysteine (fluimucil) in children is presented. It was marked, that n-acetylcysteine has significant complex mucolytic effect in children with RSD.

Abstract
His lecture is devoted to etiology, pathogenesis, and clinical presentation of celiac disease. The methods of modern laboratory diagnosis are reviewed. Special consideration is given to problem of nutrition of patients and to use of special provisions as «pregestimil». It is medical compound on the basis of hydrolyzate of casein with triglycerides.

Abstract
A problem of choice of optimal rentgenocontrast preparation for angiography and radio endovascular interventions in children was analyzed in this article. An experience of administration of iopromide in republican childrens clinical hospital in visualization of cardiovascular pathology, malformations and tumors was described. Information about application of modern technologies of angiographic diagnostics and radio endovascular surgery in children with different diseases was presented. iopromide is the well-balanced contrast media with the right mix of osmolality, viscosity and iodine concentration. Iopromide delivers the right contrast for consistently high quality imaging results.

Abstract
A problem of etiology and pathogenesis of acute respiratory infections in children are observed in this article. Modern approach to management of its treatment in pediatric patients, including often ailing children, is described. Authors give characteristics to main directions of treatment of obstructive syndrome. An experience of anti-inflammatory therapy with fenspiride (eurespal) in children of different age is summa ized in this article.

Abstract
An experience of complex approach to the problem of initial prophylaxis of atopic dermatitis on level of municipality is presented in this work. An efficacy of education of pregnant women with compromised allergic history and members of their family was demonstrated. Authors presented information about experience of address providing of children of first year of life with hypoallergenic nutrition. Decrease of incidence of atopic dermatitis in children of first year of life and increase of duration of breast feeding were the overall results of 4 year work in this sphere.

Abstract
Advisability of clinical use of l-carnitine (Karniton) and coenzyme q10 (qudesan) in adolescents is discussed in this article. An evidence of activity of these medications were based on analysis of results of clinical, functional (cardiointervalography), cytochemical investigation and special psychological testing. Combined treatment with both medications has a beneficial effect for the increase of adaptative resources of adolescents.

Abstract
Activity of probiotic medication «Bifiform» in children with juvenile arthritis treated with antibiotics is described in this article. Prophylactic application of prebiotic before beginning of antibacterial treatment in base group of patients allows avoiding antibiotic associated diarrhea. «Bifiform» stops diarrhea and other dyspeptic events in patients treated with antibiotics with manifestations of dyspepsia on average in 3 days of therapy. It's recommended to use «Bifiform» in patients with juvenile arthritis treated with wide spectrum antibiotics to prevent dyspepsia.

Abstract
The results of multicenter clinical approbation of specialized foods of «Nutrigen» line are described in this article. An assessment of activity of low protein compound for baking bread and dried protein free beverage with milk flavor was carried out in genetic centers of Moscow, Saint-Petersburg and Krasnodar. The results of a trial show that new domestic foods have an opportunity to be widely used in dietotherapy of children with hereditary metabolic diseases.

Abstract
Characteristics of treatment of cough in children with herbal medications are presented in this article. Poly therapeutic effect of medicinal plants, its high biological activity, relatively low toxicity and wide therapeutic index explain a need of phyto therapy use in treatment of cough. Advantages of phyto therapy are described, children's doses, ways of application of herbal extracts are given. The use of plants depending on the stage of disease and principles of combination of herbal medications during treatment are described.

Abstract
Inhalation therapy has significant place in treatment of different diseases in children. It has incontestable advantages over other methods. Efficacy of this method depends on choice of medication and mostly on proper carrying out of inhalation. Methods which can increase efficacy of inhalation are presented in this article, frequent mistakes are analyzed.

Abstract
The results of trials of activity of esomeprazol in children with gastroesophagial reflux disease (gerd) are presented in this article. It was marked that esomeprazol (Nexium) can be used in children of 1–11 years (10 or 20 mg daily) and 12–17 years (20 or 40 mg daily) with gerd according to modern recommendations. The information about acid suppressor activity of esomeprazol in children with gerd younger then 2 years old (0,25–1 mg/kg daily) was published. Clinical activity of esomeprazol was confirmed in a number of randomized and open labeled, developed in different countries, including Russia. At the same time main results of activity of this medication were obtained in investigations of adult patients with gerd. This fact indicates necessity of provision of wide ranging trials, which could make complete assessment of advantages and safety profile of esomeprazol.

Abstract
Case report about early debut and severe clinical course of system type of juvenile rheumatoid arthritis, refractory to treatment with classic immunosuppressive agents and blocker of tumor necrosis factor is presented in this article. A successful application of chimeric antibodies to cd20+ b-lymphocytes rituximab in dose 375 mg/m2 of surface of body weekly intravenously are described. Rituximab infusions were carried out during 4 weeks. Patient took 2 treatment courses of rituximab. Extra articular symptoms of disease and acute inflammatory lesions in joints were stopped by 8 week. Range of motions in affected joints was increased. A new attack of disease developed in 22 weeks after acute respiratory infection. Increase of level of CD 19+ and CD 20+ b-lymphocytes in peripheral blood was the reason of repeated treatment course, including rituximab in the same scheme. Treatment initiated the development of clinical and laboratory remission, duration of it was equal 22 weeks.

Abstract
Scientific center of children's health of Russian academy of medical sciences was recognized by the presidium of Russian academy of medical sciences as assignee of imperial Moscow founding hospital. The characteristics of second stage of history of scientific center of children's health, which was concerned with events, happened after grate October revolution (1917), is presented. The reorganization of Moscow founding hospital to the infants care home was developed by this short and extremely significant period (1918=1922). This home became medical, prophylactic and research and educational center, where the basis of soviet system of children's and maternity protection was laid.
