EFFECTIVENESS OF EDITING IN PREPARATION OF ORIGINAL ARTICLES: RESULTS OF RANDOMIZED CONTROLLED STUDY

Abstract
The data on effectiveness of editing in medical journals is restricted. Objective: to study an effectiveness of editing in improvement of manuscripts quality containing results of original study, which were published in the journal «Current Pediatrics» (Russia). Design: single blinded randomized controlled study in parallel groups. Consecutive eligible papers were sent to reviewers (n=137) who were randomised into 2 groups (1:1) for evaluation of five author’s or edited (accepted for publication) manuscripts. Main outcome measures: item of importance, originality, methods, data presentation, discussions and conclusion quality score were evaluated. Results: 102 (74%) participants gave at least one conclusion, 64 (47%) performed an estimation of all presented materials. 412 conclusions (208 on author’s manuscripts and 204 on edited ones) were analyzed. An analysis showed equal estimation of importance, methods, discussions and conclusions in both author’s and edited manuscripts. Evaluation of data presentation quality was higher in edited materials: odds ratio (OR) for estimation ≥ 3 according 5-point scale was 4.1 (95% CI 2.0–8.4) times higher than in author’s manuscripts. Higher estimation of editing effectiveness was received from «strict» readers (mean estimations of manuscripts quality in lower quartile). They mentioned that editing improves quality of data presentation — for estimation ≥ 3 according to 5-point scale OR was 7.8 (95% CI 2.9–20.6), conclusions — OR 2.3 (95% CI 1.0–5.2), methods — OR 2.2 (0.9–5.3; p = 0.068), and makes common impression of edited manuscripts better as well. Conclusion: editing improves quality of data presentation and description of conclusions and methods («strict» readers only) in manuscripts of original study.

Abstract
Authors presented first results of health estimation in children with airways diseases with DALY (disability-adjusted life years). The criterion is able to characterize true loss of health with differently directed dynamics of morbidity, disability and mortality of these diseases.

Abstract
Sociological study included 651 schoolchildren. Factors conductive to worsening of their health state were analyzed. Schoolchildren’s knowledge on risk factors and ways of safeguarding of their health was frequently abstract and not correlated with their behavior. Lifestyle of adolescents had some violations of normal behavior according to the age (in aspects of nutrition, physical activity, night sleep, etc.). These abnormalities were revealed in older schoolchildren more frequently than in middle-age schoolchildren. Besides, by the time adolescents leave school 25%, 30% and 7% of pupils have an experience of smoking, use of alcohol and drugs correspondingly. Adolescents are characterized with psycho-emotional discomfort, anger, aggression negatively influencing their way of communication in society.

Abstract
Neonatal screening of cystic fibrosis is able to provide early presymptomatic diagnosis of the disease in newborns. The objective of this study was to evaluate an effectiveness of neonatal cystic fibrosis screening in Moscow. Authors studied molecular-genetical samples of DNA in 1,260 newborns with hypertripsinogenemia (the level of immunoreactive tripsin in first test — IRT I was > 70 ng/ml) for the presence of most frequent mutations of gene CFTR. All children were born in Moscow in 2008. They formed risk group because of results of IRT I test. Mutations were detected in 53 persons (47 heterozygotes and 6 homozygotes). The rate of false-negative results was 0.16 %; the result was similar to European data.

Abstract
Nucleotides are used in different significant biological processes, they have prebiotic, bifidogenic, immunomodulating, trophic effect on tunica mucosa of gastrointestinal tract, they also influence lipid and carbohydrate metabolism. The literature review describes influence of nucleotides and changes of its volume in cow’s and breast milk on growth and development of children.

Abstract
The article reviews the modern data on structure and functions of intestinal micro flora. Possible ways of activation of acquired immune response and food tolerance forming are shown. Positive role of probiotics in treatment and prophylaxis of acute respiratory infections is described.

Abstract
Allergic rhinitis (AR) is global problem of world health care system. Last 50 years the prevalence of AR increased. AR influences social life, sleeping, school progress and capacity for work. AR is frequently combined with bronchial asthma; it is able to induce exacerbation of asthma. The treatment of AR should be directed to the control of symptoms, prophylaxis of complications, improvement of sleeping and quality of life of patients and their parents. Intranasal corticosteroids are the drugs of choice in treatment of children with AR, these medications stop all symptoms of the disease including eyes symptoms.

Abstract
The article presents results of systematic review of data on prophylactic and therapeutic efficacy of ibuprofen. Data search was performed by PubMed database and Google search. 27 publications for analysis were available. Prophylactic efficacy of ibuprofen was studied in 14 studies. Summarizing of the results showed that ibuprofen prevents pain and decreases its following intensity after different surgical or dental operations. There is no significant difference in prophylactic efficacy of single dose ibuprofen and acetaminophen. Therapeutic efficacy of ibuprofen was described in 13 studies. Administration of the drug for pain stopping in children is reasonable. The analgesic effect of ibuprofen compared to placebo was shown in all studies of patients with migraine and diseases of ENT-organs. 5 studies performed in last 5 years showed efficacy of ibuprofen in trauma patients, including children with non-complicated fractures of extremities.

Abstract
The results of clinical studies of bacterial lysates mixture (Imudon) efficacy in pediatric practice are analyzed. Obtained data show reasonability of the drug administration in children with recurrent respiratory diseases, pharyngeal and tonsil infections, herpetic stomatitis, acute laryngitis and tonsils extraction.

CHRONIC FUNCTIONAL CONSTIPATION IN CHILDREN — FROM UNDERSTANDING THE PROBLEM TO THE PROPER TREATMENT

Abstract
Functional origin is known in 95% of children with constipation. According to ICD-10 there is traditional distinction between syndrome of irritated bowel and proper functional constipation which have wide spectrum of causes. Rome criteria III propose initial diagnostic criteria of these disorders but the use of the criteria in pediatrics is frequently complicated. Clinical practice of pediatrician demands differentiation of constipations into hypertonic and hypotonic ones. Treatment programs include necessary component — laxative drugs which are useful in cases of subcompensated and decompensated types of constipation. One of the laxative drugs is Forlax containing polyethyleneglycol 4.000; it can be given to children 6 months old and older. The literature data shows high efficiency and safety of the drug.

Abstract
The theme of the article is one of the most wide-spread rheumatologic diseases — juvenile arthritis (JA). It highlights main questions on epidemiology, classification, pathogenesis of the disease. Authors describe clinical symptoms of different types of JA, diagnostics and differential diagnosis. There is a part of an article with information on drugs used in patients with JA and the algorithm of differentiated treatment of a disease is presented. Authors paid special attention to approaches to biological therapy, conditions for the treatment and management of patients treated with genetically engineered biological drugs.

Abstract
Pneumococcal infection induces both severe invasive diseases and widespread non-invasive disorders of respiratory tract; it can cause recurrent acute respiratory infections (ARI) and their complications. Present article analyzes experience of treatment with pneumococcal polysaccharide vaccine Pneumo 23. Authors studied morbidity with ARI (according to medical documentation) during 6–12 months after vaccination in children 2–14 years old with different pathologies. The study showed high efficacy of vaccination in frequently ailing children and patients with allergic diseases.

Abstract
The article analyzes an experience of nutrition in children with different disorders (infectious and not infectious diseases) with adopted mixture «Agusha Gold 1». Mixed feeding with dried adopted mixture provides stable increase of child’s body mass. This effect is related to optimal composition of mixture and inclusion of probiotic cultures and prebiotic fibers in it.

Abstract
One of necessary conditions in treatment of children with allergic diseases is eliminational hypoallergenic diet. Endogenous and exogenous vitamin deficiency is a result of the dietotherapy. Administration of vitamins and minerals complexes is necessary in children with allergic pathology. The choice of supplementary drug should be made according to character and severity of disease, composition of drug, its form of delivery. The article reviews an experience of vitamins and minerals complexes administration in children with different allergic disorders.

Abstract
Problem of nutritive status correction in children with body mass deficiency is still topical. Drug treatment is not efficient completely is no individual correction of nutrition is provided. Development of a diet should be performed taking into account peculiarities of infants with body mass deficiency. Correction of nutrition in 20 children with hypotrophy and 20 children with unstable stool was performed with Vallings — liquid homogeneous milk-cereal mixtures with high nutritive value and sparing consistence. Normalization of intestinal function and common increase of body mass was detected in one month after the beginning of treatment.

Abstract
Acute respiratory infections (ARI) are widespread in children regardless of age and region of living; they are characterized with big amount of infectious agents and absence of a trend to morbidity decrease. Drugs for nonspecific prophylaxis (immunostimulators and immunomodulatory agents) are frequently used for prevention of ARI. There are plenty of immunomodulating agents; the wellstudied medication with systemic action with good efficacy and safety in pediatric practice is ribosomal-proteoglycan complex. The article presents the description of clinical experience of treatment with this complex in pediatric practice.

Abstract
The article analyzes the role of pneumococcal infection as a cause of complications and death in patients with influenza. Authors performed historic analysis of data on development of bacterial complications in period of influenza pandemic. Possible pathogenetic mechanisms of bacterial co-infection development are characterizeed. Effective vaccination of children with pneumococcal conjugated vaccine is able to decrease morbidity and mortality in common population and in group of high risk patients.

Abstract
The article discusses main questions of diagnostics of diarrhea in children. Main cause of acute diarrhea is infection, mainly viral (rotavirus, etc.). Chronic diarrhea frequently has non-infectious origin. The need of multi-aspect diagnostics of diarrhea cause in children is related to the significance of treatment of main disease. Besides, treatment of chronic and acute diarrhea include major component: adsorbents based on smectite. In total treatment of diarrhea has to be complex with the use of dietotherapy and medications: mucocytoprotectors, regulators of motoric, pre- and probiotics.

Abstract
One of effective and safe immunomodulators for prophylaxis and treatment of frequently ailing children is pidotimod (Imunorix). Efficacy of the drug in pediatric practice was studied in more than 3200 patients with acute and recurrent respiratory infections. The article shows reasonability of pidotimod administration in children with acute and chronic bronchitis. This fact was confirmed with doubleblinded placebo-controlled studies. Treatment with pidotimod results in decreased terms of recovery of chronic bronchitis exacerbation, shortening of exacerbation. Realization of stable effect is related to recovery of key functions of inborn and adaptive immunity, it begins in 15 days after intake of the drug in therapeutic dose. Prophylactic doses of pidotimod should be used during next 30–60 days.
