Abstract
Review is devoted to the prevalence of smoking among children and adolescents, given the steady increase in illness, a risk factor which is smoking. Detection disease at a stage before the disease, clinical manifestations, requires the implementation of demonstration and preferably noninvasive studies of children. Shown that early intervention in the prevalence of tobacco use would decrease the disease from respiratory and cardiovascular systems, and hence improve the quality of life of future generations.

Abstract
Breast feeding is «the golden standard»of childern’s nutrition on their first year of life. Basic knowledge in this field is given to future medical professionals throughout their years of study in a medical school. Study objective: to assess whether knowledge and skills that future doctors possess in the field of maintaining of breast feeding is adequate to modern requirements. Methods: as a part of retrospective cross-section study a questionnaire was given to medical students of two final years as well as to interns and residents.  Results: in 2010–2011 the total of 1088 medical students from 8 medical schools and Novokuznetsk Institute of Advanced Medical Studies was questioned. 64.9% from the total were from pediatric faculty and 35.1% on a general medical faculty. The proportion of correct answers to theoretic questions was more than 85%. However the majority of students experience difficulties with implying this knowledge to solve practical cases. No major differences in replies given by pediatric and medical students were found. Conclusion:  The «alienation» of theoretic knowledge that students possess from its practical implication doesn’t help to solve problems with maternal lactation. Therefore it’s necessary to pay more attention to practical implication of theoretical knowledge given to medical students, especially upon infantile nutrition.

Abstract
Nowadays varicella infection is important to the public health specialists due to not only its high morbidity rates, but also the existence of severe forms that can lead to serious complications and even death. The overall damage caused by varicella infection and its’ complications in the Russian Federation is a multicomponent and a multiple-factor figure, that reflects considerable annual economic damage caused by this infection. In this article one can find a substantiation of the suitability of varicella infection immunization on a national scale.

Abstract
The study objective was to assess efficacy and safety of etanercept treatment in combination with methotrexate in 25 patients with juvenile idiopathic arthritis. In this study were included 25 children with poly- and oligoarticulate variants of juvenile idiopathic arthritis aged 8 (6; 11) years. In the beginning of the study 17 patients had poliarticulate variant, 8 children — oligoarticulate. High clinical activity of the disease was accompanied by general inflammatory reaction. Analysis of the results has shown that etarnecept treatment ensured statistically significant and trustworthy positive dynamics of clinical and laboratory indices of disease activity. After 6 month of treatment 59% (13 from 22) of patients were diagnosed with inactive disease, after 12 months — 86% (19 from 22) of patients had this status. Etanercept treatment was well tolerated and the majority of side effects were minor or medium, reversible and caused no interruption of the treatment.

Abstract
The article contains review of Russian research results dedicated to clinical efficacy and safety assessment of topical bacterial lysate, used with therapeutic and prophylactic purpose in children with different premorbid background, including children from welfare institutions. After a course of prophylactic treatment with topical lysate it was proven for certain that prevalence of acute respiratory infections during the seasonal increase of sickness rates as well as exacerbation of chronic ENT and respiratory pathology was lower. It has been shown reasonable to perform refresher courses of treatment with the medication in order to prevent nidiformation in nasopharynx.

Abstract
The article contains analysis of international practice of abatacept use in treatment of refractory uveitis in patients with idiopathic juvenile rheumatoid arthritis. Efficacy of T-cell co-stimulation blockade in treatment of severe autoimmune fontal uveitis has been proven on the models. Described clinical cases show that, abatacept is an effective and well tolerated biological drug, that allows achieving success in treatment even after proven inefficacy and intolerance of traditional immunosuppressants and TNF α antagonists.  Shown results testify to availability of a abatacept treatment of a refractory uveitis in patients with juvenile idiopathic rheumatoid arthritis and the necessity of further efficacy and tolerance research with protracted therapeutic schemes.

Abstract

Abstract
Traditional approach to cystic fibrosis patients treatment doesn’t involve upper respiratory tract assessment, though abnormal changes — consequences of the cystic fibrosis transmembrane conductivity regulator gene mutation- do affect nasal and paranasal mucosa to the same extent. Approximately half of cystic fibrosis patients suffer from chronic rhinosinusitis and/or nasal polyposis that worsens the clinical course of already severe disease. Chronic hyperplasia in paranasal cavities can be quite extensive, recurrent and can lead to destruction of osseous walls of the cavity and of nasal septum. Thus increasing the amount of hospital admissions and and their duration. Low awareness of ENT-specialists working in polyclinics and in hospitals of ENT-pathology in cystic fibrosis patients leads to belated diagnostics, excessive manipulations, ineffective treatment, including surgery. All these lays grounds to implication of the early screening diagnostic program and development of proper treatment methods of ENT-complications of cystic fibrosis — therapeutic as well as surgical, with strict specification of indications and contraindications.

Abstract

Abstract
Infliximab is one of the first biological agents that has proven its efficacy in Crohn’s disease treatment in various multicenter randomized studies. Anticytokine treatment with infliximab allows to reach clinical, lab and endoscopic remission and secure its maintenance. This article contains clinical examples of anticytokine therpy with infliximab (Remikeid) efficacy in children with Crohn’s disease.

Abstract
Among pediatric allergic diseases, atopic dermatitis takes one of the leading positions. Functionality of gastro-intestinal tract plays quite an important role in its development. Gastro-intestinal lesions first of all lead to the damage of protective mucosal barrier thus leading to the damage of intestinal barrier function. Due to this, therapy aimed at allergen and toxin elimination plays an important part in complex treatment of atopic treatment. The most effective are sorption methods. In pediatric atopic dermatitis guidelines enterosorbents are included along with elimination diet, local treatment and antihistamines. In the recent years dioctahedric smectitis (Neosmectin)  is in quite active use. This article contains results of the research upon its use in complex treatment of atopic dermatitis in children.

Abstract
Surgical treatment of congenital heart disease is quite often accompanied by total or partial resection of thymus, due to heart access.  Thymectomycan lead to various immunological abnormalities. 43 children aged 3 to 6 years, operated due to congenital heart disease with partial or total thymectomy, took part in the research. Immunological status was measured using tests of 1 and 2 levels after 1 and 2 years from the surgery. β-carotene treatment had positively influenced on changes in immunological status of children that underwent thymectomy, and that proposes immunomodulating activity of the medication under consideration and its use as a nonspecific immunomodulator in children.

Abstract
Metabolic abnormalities in metabolic syndrome affect the functioning of practically all organs and systems, and most seriously — cardio-vascular system. Cardio-vascular abnormalities in metabolic syndrome manifest as arterial hypertension, Riley-Day syndrome and endothelial dysfunction that can lead to decrease of adaptive and reserve capabilities. Co-enzyme Q10 possesses cardioprotective,  stress-protective and anti-ischaemic activity. Clinical study performed on 40 children aged 10 to 17 years with constitutive obesity, complicated metabolic syndrome, has proven validity of co-enzyme Q10 treatment in patients with metabolic syndrome. The use of co-enzyme Q10 15 mg/day during 30 days has lead to improvement of psycho-emotional condition, decrease in anxiety complaints, sleep improvement, decrease in asthenic syndrome symptoms, improvement in electrophysiological heart indices

Abstract
Respiratory pathology and acute respiratory infections make the major part of all pediatric diseases. This pathology is a matter of special importance especially in the group of frequently-sick children. This article contains information upon functional peculiarities of immune system, necessity of effective preventive measures development, or if protracted clinical course and treatment occur, recurrent respiratory diseases, as well as immunization of such children against the background of immunocorrection with ribosomal vaccine.

Abstract
The article is dedicated to the problem of rational nourishment of nursing mothers. It contains approximate daily set of nutrients and recommended norms of main nutrients and energy intake during lactation. This article also underlines the necessity of enriching women’s diet during lactation with essential functional nutritional factors and thus the possibility of use of enriched products for pregnant and nursing women. Also there’s information about contents of Russian specialized formula «Annamaria — Prima for nursing mothers», its functional properties, as well as our own clinical observations of mothers, who were receiving this formula.

Abstract
Broad spectrum of antiviral activity of interferons lays grounds to its use in complex therapy of acute viral infections. It has been shown that antiviral and antibacterial interferon activity reveals itself while used both systematically and locally. Clinical efficacy of locally applied viferon is accompanied by nasal mucosa cytokine status changes, as has been shown in nasal swabs. There has been shown dependency of local immune response on the age, severity of the disease, clinical forms of respiratory syndrome, bacterial complications, if present, intensity and characteristics of viral contamination against the background of viferon treatment.

Abstract
The article is dedicated to the problem of acute rhinosinusitis in children. It contains data from our own research upon hypertonic solution efficacy and suitability in complex therapy of rhinosinusitis. It was shown that inclusion of hypertonic solution of sea water spray into treatment scheme of acute rhinosinusitis allows to reach amelioration of nasal functions and patient’s well being in short term, reduces total duration of the disease, thus decresing the risk of complications development.

Abstract
New approach to optimization of feeding of infants on their first year was regarded in this article. The role of probiotics in children’s health maintenance has been underlined in the article as well. Also there’s new information upon efficacy of a milk formula enriched with lactobacteria (Lactobacillus fermentum hereditum CECT5716), extracted from breast milk, in nourishment of infants receiving artificial formulas.

Abstract
Fever is the commonest symptom of pediatric diseases. During first two years of life the child usually experiences about 4–6 fever episodes that are normally cause of enormous parental concern. That is why fever is one of the commonest reasons of calling a pediatrician, and it’s quite natural that physicians tend to prescribe anti-fever medications. Due to mass consumption of various febrifuges it’s quite important to choose the safest of them, as well as informing doctors and parents about certain cases when febrifugal treatment is necessary.

Abstract
Periodic fever — is a hereditary autosomal — recessive disease that is characterized by recurrent serositis and frequent amyloidosis development. One of the most common symptoms of this disease is abdominal pain. This article contains modern approach to pathogenesis, diagnostics and treatment of periodic fever. There is also a correlation analysis of dependence of periodic fever clinical course from gene MEFV mutation, as well as practice of selective M-cholinergic antagonist gioscine buthilbromide for abdominal pain relief in children and adolescents.

Abstract
Inflammatory disorders of palatine tonsils are quite common in children. Usually complex treatment schemes of paratonsillitis include sprays, mouth rinses, soluble tablets and lozenges that contain various anti-inflammatory components. Their use decrease pain and duration of treatment, but doesn’t exclude the use of systemic anti-inflammatory medications.

Abstract
Intestinal microflora derangement is quite common among children with various somatic abnormalities. Jne of the most common reasons for disbacteriosis development — antibiotic therapy. The vast majority of antibiotics affects gastrointestinal microflora that leads to dyspeptic syndrome development and so-called antibiotic-associated diarrhea. Intestinal disbacteriosis affects homeostasis, leading to considerable shifts in the state of different systems and organs. That’s why correction and prophylaxis of intestinal disbacteriosis against the background of antibiotic treatment is quite important for a pediatrician. This article contains main information upon epidemiology, pathogenesis, clinical forms and clinical presentation, diagnostics and treatment of antibiotic-associated diarrhea among children.

Abstract
In the past years we have gained enormous success not only in the field of understanding of pathologic basis and genetics of cystic fibrosis, but we have also developed new therapeutic approach to this disease. Enzyme therapy is one of the main parts of a complex treatment of cystic fibrosis. Correct therapeutic decision upon medication and treatment regimen allows to reach dramatic amelioration o patient’s condition, and significantly decrease the risk of complications of the disease itself and side-effects of the treatment given. This article contains guidelines of enzyme therapy in cystic fibrosis and clinical mistakes analysis that occur while deciding upon therapy.

Abstract
The article contains clinical case description of a severe systemic juvenile rheumatoid arthritis, that was refractory to classic immunosuppressant therapy. The disease was characterized by such extraarticular manifestations as fever, lymphadenopathy,  hepatosplenomegaly, polyserositis, generalized joint involvement and high activity in lab tests. As a result of severe clinical course of the disease, patients develop bilateral aseptic bone necrosis in coxofemoral joints and coxarthrosis. Against the background of glucocorticosteroid treatment the patient has developed hormone-dependency and hormone resistance. Inclusion into the treatment of anti-CD20 monoclonal antibodies (rituximab) has stopped systemic manifestations of the disease, inflammation in the joints, normalized lab activity rates. The positive therapeutic effect allowed to perform surgery due to bilateral coxarthrosis. These results show that rituximab is highly effective in children with systemic juvenile rheumatoid arthritis, that is resistant to classic immunosupressants and glucocorticoides.

Abstract
Arteriovenous malformation — is a congenital vessel abnormality that is characterized by pathologic arterial and venous plexuses formation within the brain without capillary web between them. These malformations can have no clinical presentation or can reveal themselves as intracranial haemotomas against the background of subarachnoid and/or intraventricular hemorrhage. This article contains clinical case of a boy with persistent functional hyperprolactinemia with congenital brain vessel abnormality, also we discuss various possible pathogenetic mechanisms of hyperprolactinemia against the background of arteriovenous malformation.

Abstract
This article contains observation of severe poliarticular variant of juvenile rheumatoid arthritis, resistant to glucocorticoid treatment and classic immunosuppressive therapy. There is a description of biologic treatment — with chimeriс antibodies to TNF α — infliximab in a patient with protracted aggressive arthritis (duration of the disease more than 5 years). After 6 weeks of treatment there was a considerable decrease in pain syndrome and joint exudation, morning constraint was arrested completely. After 30 days of treatment an inactive phase of the disease was achieved, movements were restored in 15 from 23 affected joints. We managed to stop intraarticular injections with glucocorticoids. Clinical case shown can serve as an example of high efficacy of infliximab in patient with protracted juvenile rheumathoid arthritis and testifies to the availability of a further treatment of patients with aggressive juvenile rheumatoid arthritis, that are resistant to classic immunosuppressive treatment, with TNF α blockers.
