Abstract
The article represents organizational - functional pattern of optimization of social psychic and medical pedagogical assistance for native children and adolescents in Tyva Republic. The pattern is aimed at psychic health improvement and prophylaxis against non-adaptable forms of conduct. It is based on complex approach to providing assistance to children and adolescents, preventive measures and the increase in the quality of medical assistance.

Abstract
The results of evaluation of quality of life in children from 1 to 3 years old with chronic diarrhea in exacerbation or remission with QUALIN questionnaire is presented in this article. Authors used two types of questionnaire: doctors' and parents'. Obtained data was compared to indices of healthy children. Proved, that children in early age with chronic diarrhea, as in exacerbation, as in remission, have significantly lower rates of quality of life compared to healthy coevals.

Abstract
The objective of open-labeled study was estimation of effectiveness and safety of infliximab — monoclonal antibodies to tumor necrosis factor (TNF) - in treatment of 48 patients with juvenile ankylosing spondylarthritis. Duration of observation was 6 weeks — 1,5 years. Anticytokine treatment was administrated on the ground of therapy with immunosuppressive agents in 96% of patients. Infliximab was administrated in median dose 7,4 ± 3,7 mg/kg of body weight by standard scheme (0–2–6 — week and further every 8 weeks) intravenously. Results of a trial showed that infliximab has evident anti-inflammatory effect. Treatment with infliximab provided development of remission of articular syndrome, decreasing and normalization of laboratory indices of activity of disease, the rate of disability, and increasing of quality of life in 84% of patients. Effect of this medication was registered after first infusion and continued during all period of follow up. Adverse effects included transfusion reactions: fever, head ache, nausea/vomiting in 10% of patients, allergic arthritis — in 2% of patients. Thus, treatment of infliximab is pathogenetically grounded, effective and safe in patients with juvenile ankylosing spondylarthritis.

Abstract
This trial studied effectiveness and safety of pidotimoid (Imunorix) in complex treatment of children with acute respiratory infection (ARI). Treatment with pidotimoid during 2 weeks (n = 30) resulted in lesser duration of fever and intoxication symptoms, and symptoms of laryngo-tracheitis, compared to control group (n = 30). Besides, children from pidotimoid group showed more rapid transformation of dry cough to hydrated cough, and decrease of its intensity. This beneficial change was accompanied by improvement of microbiocenosis. Effectiveness of pidotimoid was estimated by 73% of doctors as «good» (67% in control group). There was no any complication, related to treatment with this medication. The rate of repeated cases of ARI was three times lower then in control group in 6 months. All patients with ARI had no recurrent laryngeal stenosis.

Abstract
The results of treatment of children (n = 64) of 1,5–17 years old, receiving rehabilitation course after reconstructive — restorative surgery of bowels are presented in this article. It was shown, that prolonged (1 month and more) administration of lactulose (Duphalac) results in improvement of motor and evacuation function of bowel, and decreases «sensor inability» of reduced parts of bowel. As a result, lactulose stimulates functioning of «obturative apparatus of neoampula» (voluntary evacuation, retention of intestinal contents), that favors to increase of quality of life and social rehabilitation of children in outlying post surgical period.

Abstract
Problem of prophylaxis and treatment of early anemia of prematurely born infants is still very important. It's related to increase of rate of infants, surviving with very low and extremely low body weight at the time of birth. The purpose of trial was to provide an optimization of scheme of treatment of early anemia with recombinant human erythropoietin (rh-EPO, Recormon). This trial included 94 infants with very low and extremely low body weight at the time if birth. 26 infants was treated with rh-EPO 500 ME/kg once a week, and 33 infants — with rh-EPO 1000 ME/kg once a week, 35 infants was in control group. Rh-EPO was administrated by subcutaneous introduction once a week, from 3–5 day of life during 6 week. All patients receive iron in dose 4 mg/kg daily. As a result, the need in transfusion was decreased 5 times lower in group of children receiving rh-EPO 500 ME/kg weekly compared to control group. Patients, receiving rh-EPO 1000 ME/kg weekly, had no need in hemotransfusions.

Abstract
This article presents results of a trial, studying provision with vitamins in younger schoolchildren from Orenburg. This research showed low intake of vitamins D, C, B1 and folic acid. Authors proved that schoolchildren have combined nutritive vitamin deficiency and gave estimation to content of vitamins in urine.

Abstract
Violent growth of type 2 diabetes mellitus (DM) morbidity in children and adolescents is spread all around the world. Peculiarities of DM type 2 in this age group are: absence of determined diagnostic criteria, difficulty of differential diagnosis with DM type 1, early development of complications. International Society of Pediatric and Adolescent Diabetes propose Guideline of DM type 2 management, which consider main questions of diagnostics, treatment and prophylaxis of this disease. The agent of choice for treatment of DM type 2 in children upwards 10 years is metformin.

Abstract
The role and mechanisms of realization of biomechanical, physical, chemical and biological factors of system of non-specific defense of organism are discussed in this article. Authors characterize the main cell and humoral factors of this system; analyze etiology, pathogenesis and main signs of syndromes of insufficiency of immunosurveillance of organism, developing as a result of disorders of inborn immunity mechanisms.

Abstract
This article describes actual problems of abdominal pathology in children: irritable colon syndrome and constipation. The questions of forming those state, criteria and standards of diagnostics and treatment, questions of differential diagnostics are discussed. Abstract
Pericarditis is relatively widespread inflammatory disease of pericardium. Symptoms of this disease was described already in the beginning of XIXth century, but it's clinical diagnostics in patient's lifetime is difficult up to now. Majority of acute pericardites have favorable clinical course, but some of them have complications (cardiac tamponade and constrictive pericarditis), which are potentially lethal. Development of non-invasive radiation methods of heart visualization technologies improved diagnostics of pericarditis in last years. Besides, achievements of immunology, virology and microbiology allowed widening of data of etiology and pathogenesis of pericardial diseases. Present lecture gives modern data of etiology, classification, clinical signs, diagnostics and treatment of pericarditis. The description of separate specific types of pericardites is given.

Abstract
The opportunities of receiving of genetically engineered medications, e.g. somatotropic hormone (STG) are almost unrestricted, and treatment and monitoring of patients with different types of microsomia can be held on modern, new level with the help of it. STG provides normal stature and valuable quality of life in these patients. Treatment with growth hormone influences on hormonal, metabolic and psychical status of patient. Metabolic effects are: increasing of muscle strength, improving of renal blood flow, increasing of cardiac output, absorbability of calcium in intestines and mineralization of bones. The level of blood cholesterol, lipoproteins is descended; blood alkaline phosphatase, phosphorus, urine and fatty acid are increased. Patients' vitality and quality of life are normalized. Besides somatotropic insufficiency, growth hormone is widely used in growth_stimulating treatment of different types of dwarism in children: microsomia due to pre_natal growth delay, genetic syndromes: Silwer–Russell, Shereshevsky–Turner, Nunan, Prader–Willi, and microsomia in patients with chronic renal disease.

Abstract
This lecture presents modern approach to treatment of disorders of intestinal microflora and, as a result, the reasons of indigestion. A contribution of dysbiosis to alteration of intestinal transition, disorders of nutritive tolerance and development of allergy was marked. Special attention was given to analysis of active enterosorption — a pathogenetic method of correction of endotoxinemia, which can be reflection of microbe misbalance.

Abstract
This article presents results of a trial, studying quality of life of adolescents with multiple sclerosis. Obtained data show, that adolescents had decreased quality of life, including physical and psychosocial functioning. Decreasing of total rate of quality of life was concerned with, first of all, disorders in school and physical functioning, and emotional constituent was intact. It was shown, that pathogenetic treatment with interferon beta1? (Rebif) did not significantly decreased psycho-emotional constituent of quality of life. On the contrary, patient in remission had increase of rates of psycho-social health on the basis of insignificant decrease of physical and emotional indices, ant that caused increase of summary rate of quality of life.

Abstract
This article presents a data of role of normal micro flora of gastrointestinal tract in forming of alimentary functions and immune system of newborn. Processes of formation of intestinal micro biocenose, and pre and postnatal factors of development of dysbacteriosis in children of early age were analyzed. Authors give modern classification of probiotics. Results of trials show effectiveness and safety of combination of probiotiс cultures Bifidobacterium lactis BB-12 and Streptococcus thermophilus, forming new complex «Bifiform Baby» for children of early age.

Abstract
Excretory pancreatic insufficiency is relatively frequent pathology in pediatric and therapeutic practice. It can be divided to primary and secondary, inborn and acquired, absolute and relative one. The causes of primary pancreatic insufficiency are agenesia or hypoplasia of pancreas, cystic fibrosis, Shwachman–Diamond syndrome. Secondary excretory pancreatic insufficiency can be caused by resection of pancreas, pancreatitis, pathology of mucous tunic of small intestine, abnormality of antitrypsinactivation with enterokinase deficiency, inactivation of enzymes by acid of duodenal contents in the presence of gastric hyper secretion and others. Excretory pancreatic deficiency has determined clinical signs and can be relatively easily diagnosed by simple laboratory methods: coprogramm and detection of elastase 1 in stool. The ways of its correction are determined by causes of disease, but modern high-active pancreatic enzymes medications must be necessary component of treatment in all cases.
